Description
We are seeking a knowledgeable and compassionate Nutritionist to join our team at Riverside Family Physician's Enhanced Management Program. This position is crucial in supporting our patients' health through personalized nutrition plans and education.
Responsibilities:
- Conduct nutrition assessments and develop customized meal plans for patients based on their health goals, medical history, and dietary preferences.
- Provide one-on-one counseling and ongoing support to patients to promote healthy eating habits and lifestyle changes.
- Collaborate with our multidisciplinary team of healthcare professionals, including physicians, nurses, and therapists, to integrate nutrition therapy into comprehensive patient care plans.
- Conduct group nutrition education sessions to address common health concerns and promote preventive care.
- Stay updated on the latest research and guidelines in nutrition and wellness to ensure evidence-based practices.
Requirements
- Bachelor's degree in Nutrition, Dietetics, or a related field; Master's degree preferred.
- Certified Nutrition Specialist (CNS) or Registered Dietitian Nutritionist (RDN) credential is required.
- Minimum of 2 years of experience in clinical nutrition or a similar setting.
- Strong interpersonal and communication skills with the ability to build rapport and motivate patients.
- Proficiency in using nutrition assessment tools and software for meal planning.
- Knowledge of chronic disease management and preventive nutrition strategies.
- Ability to work independently and as part of a collaborative healthcare team.
